Group Medical Visits (GMV)

Group medical visits offer a new and time-efficient way of caring for patients who share a specific condition, such as diabetes. Patients with a similar chronic condition are invited to attend these sessions to receive care, education, and advice within a supportive group environment.

Although individual appointments for such patients are still required, many longer-term health maintenance concerns for these patients can be more effectively addressed through group visits.  A survey of physicians who have completed this learning module revealed 96% believe group visits have resulted in their patients being more involved in the self-management of their care, and 91% said group visits helped increase patient satisfaction.

Physicians and their staff participating in this module learn to plan and implement group visits and group medical appointments. Specific goals for physicians completing the module include holding at least one group visit every month, and including another health care professional (e.g. a nurse or dietitian) for some of these group visits.

Patient Self-Management (PSM)

Enables GPs and their staff to help patients take a bigger role in managing their own health. This requires building the patient’s confidence in their ability to change and to adopt healthier behaviours. Physicians learn how to help patients identify behaviours they are prepared to change and how to help them develop a plan for changing those behaviours, one step at a time.

Through this module, physicians learn how to help patients identify behaviours they are prepared to change and how to help them develop a plan for changing those behaviours, one step at a time.

Physicians will also learn how to:

  • assist patients in setting goals for self-directed changes of behaviour
  • provide consistent evaluation of patient self-management plans
  • educate patients about healthy lifestyles and behaviours. 

In a survey of physicians who have completed this module, 81% said their patients seem to like self-management, and 93% said they planned to make self-managed care an ongoing part of their practice.

 

PSM Learning outcomes

After completing the Patient Self-management module, you and your MOAs will be able to:

  • help patients set goals to facilitate self-directed behaviour change
  • provide patient support for solving daily problems
  • conduct consistent patient evaluation and follow up of self-management plans
  • educate patients on healthy lifestyles and behaviours.
