Chronic Disease Management
The Chronic Disease Management Incentives (14050, 14051, 14052, 14053) provide funding for GPs to not only identify chronic illnesses such as diabetes, hypertension, and congestive heart failure but also to develop a care plan and work with flow sheets and registries to manage and improve patient care.
This incentive program recognizes the additional work, beyond the office visit, of providing guideline-informed care to patients with the following chronic conditions:
- diabetes mellitus
- congestive heart failure
- chronic obstructive pulmonary disease (COPD)
